Separations
DD Form 214 FAQs
The DD Form 214 is the discharge certificate or certificate of release from active duty. Its purpose is to provide the officer, the Department of Veterans Affairs, and other government agencies with a clear, concise summary of a person’s active duty and reserve component service as applicable, at the time of transfer, release, discharge, or change of component or status. This includes relevant data regarding the Service member’s service and the circumstances of termination. It is an authoritative source of personnel information for administrative purposes and for making enlistment or reenlistment eligibility determinations.
CCHQ has worked closely with our Department of Defense (DoD) counterparts and USPHS Commissioned Corps now has permission from the DoD to use this form. A May 24, 2016 memo from acting Undersecretary of Defense, authorized the use of DD Form 214. On August 10, 2020, the Assistant Secretary for Health approved the issuance of DD Form 214 and 215. Moreover, DoD INSTRUCTION 1336.01, states that this issuance applies to “The United States Public Health Service Commissioned Corps, by agreement with the Department of Health and Human Services, unless specifically indicated in this issuance.”
The DD Form 214 is used whenever a PHS officer separates from active duty status. It is the standard record of service for members of the Regular Corps and Ready Reserve Corps and is used by:
- Service members who are separated or released from active duty.
- Recalled retirees reverting to retired status, regardless of the period of active duty service.
- Service members who are separated for cause or for physical disability, regardless of the period of active duty service.
- Personnel being separated from a period of active duty for training, full-time training duty, or active duty for operational support when they have served 90 days or more, or when required by the Secretary concerned for shorter periods.
- Reserve Component personnel ordered to active duty for deployment for a period greater than 30 days.
- Service members who change their status or component while serving on active duty.
- Reserve officers who are issued a DD Form 214-1.
- As prescribed under other circumstances determined by the Uniformed Service concerned.
The DD Form 214-1 is only for reservists. The DD Form 214 is for both. The DD Form 214-1 will always be accompanied by the DD Form 214. A DD Form 214-1 is issued for:
- Service members who are separated or released from a Reserve Component.
- Reserve Component Service members transferred to the Individual Ready Reserve or the Inactive National Guard.
- Reserve Component Service members transferred to the Standby Reserve or Retired Reserve.
- Recalled retirees reverting to retired status.
- As prescribed under other circumstances determined by the Uniformed Service concerned.
Yes, DD Form 214 is used for any separation or release from active duty.
Yes, you get one when you leave Regular Corps and will be issued a DD Form 214 (and DD Form 214-1) as applicable when member of the Ready Reserve Corps.
Officers do not receive a DD Form 214 if:
- Found disqualified upon reporting for active duty and who do not perform duties in accordance with orders.
- Were removed from the Temporary Disability Retired List.
- Received temporary appointments to warrant officer or commissioned officer grades.
- Had their temporary warrant or commissioned officer status terminated and who remain on active duty to complete an enlistment.
- Had separated or discharged with a prior edition of this form, unless that form is in need of reissuance for some other reason.
- Were dropped from the rolls in accordance with Section 1161 of Title 10, U.S.C.
At this time, the DD Form 214 is only available for officers retiring or separating in the future; CCHQ will not replace existing statements of service with DD Form 214 at this current time. We are working with OASH Office of the Information Technology Officer and security to assess if this is feasible in the future.
The DD Form 214 covers more information on your active duty and reserve records (i.e., awards, badges and citation, your home of selection, last duty assignment, net active service, total inactive service, days accrued leave paid, character of service, agreement to share information with Veterans Affairs, etc.) It is more automated and provides information that agencies and Departments can use to adjudicate separation benefits.
PHS officers can initiate the DD Form 214 process three months before retirement or separation date.
Your separations counselor will initiate the DD Form 214. Once the form is ready for your review, you will receive an email notification. Please log into Commissioned Corps Management Information System (CCMIS), and enter the forms section labeled “forms to review”. You will then enter your information in the required fields. The forms platform should be very familiar to you as it is the same platform you use to begin your Commissioned Officers’ Effectiveness Report and Practice Hours forms.
This is an electronic form so the system will automatically route it to appropriate personnel after the officer has filled in the required fields. Once the separation counselor fills out the form, the initial form will be mailed to you with your separation packet.
After the initial hard copy is sent to you via postal mail, any subsequent copies will be sent electronically.
Yes, but officers who are no longer in the USPHS Commissioned Corps cannot access their eOPF directly.
Yes, the Separations Counselor can amend the signed DD-214 if there is an error or correction.
After an officer’s separation or retirement, the Separations Team will mail the original DD Form 214 to the former officer. Officer will confirm the forwarding mailing address before signing the DD Form 214.
PHS officers who are separating or retiring from Active Duty in October 2021 and beyond will receive the DD Form 214.
An e-mail can be sent to phsdd214@hhs.gov to request a copy of the DD Form 214. Upon request by the officer, the Separations Counselor/Specialist will be able to retrieve the DD Form 214 from the officer’s eOPF and forward it to the officers via encrypted email.
